**IMPORTANT MESSAGE FOR YEAR 4 PARENTS AND CARERS FROM CHURNET VIEW**

For the attention of Year 4 parents and carers:
As many of you will be aware, this year we have needed to try a new way of completing the data collection forms due to being unable to deliver packs to each feeder school. Unfortunately, due to the amount of settings that are now Microsoft Forms for data collection, and a bug in the Microsoft software, only a small section of our data collection form was displayed on the website. We have spoken to Microsoft today and they have informed us that the bug has now been fixed. However, we do not currently have the data that we require to input it on to our systems. Therefore, please could you complete the data collection form again at your earliest convenience. This has now been separated into two sections – Section A and Section B. Please complete both sections. We sincerely apologise for the inconvenience that this may cause.
If you have completed a paper pack, you do not need to complete this again.
